    After disabling SIP, and going through the other steps on here, I realized that the actual issue was that my mid-2012 MacBook Pro connected to an LG 34UM68-P 34-Inch 21:9 UltraWide via Mini DisplayPort to HDMI didn't actually support 2560 X 1080 60Hz. I can get the highest resolution and frame rate combination to work through SwitchResX, but as soon as I reboot the Mac Mini, after the Apple logo loading bar screen passes, the screen flickers and flashes green. I bought a Club 3D mini DP to HDMI adapter and downloaded/bought SwitchResX to unlock and utilize the higher resolutions and refresh rates.
